To make your travel reservations, follow these steps:

Step 1: Review the ESI Travel Guidelines
Step 2: Request travel arrangements using the Delegate Travel Arrangement Profile (TAP) Form.
Step 3: Upon receiving a tentative itinerary, contact the ESI Travel within 24 hours to confirm your itinerary.

If you would prefer to download the Delegate Travel Arrangement Profile Form (PDF document), please print it, fill it out, and fax it to Pat Rice at 205-967-7172. You will then be contacted by an ESI Travel agent to complete your travel arrangements. To open this printable form, you must have Adobe Acrobat Reader installed on your computer.
